Abbott Laboratories is continuing its Future Well Communities program, an initiative designed to address complex social and economic factors contributing to health inequities. The program focuses on tackling the barriers that prevent individuals, particularly in underserved communities, from managing chronic diseases effectively.

Launched in Stockton, California, in 2019, the program aims to improve access to healthcare, education, and economic opportunities. Abbott highlights that chronic conditions such as heart disease and diabetes disproportionately affect low-income communities and communities of color, leading to significant health disparities that can be passed down through generations.

The initiative employs a community-based approach, providing tailored support services. These include transportation to medical appointments, access to healthy food options, and health education, with a specific focus on diabetes management. During the COVID-19 pandemic, the program adapted to offer expanded virtual services and targeted support for vulnerable populations.

Future Well Communities has refined its strategy to prioritize neighborhoods in Stockton most impacted by diabetes and other diseases. The program has set a goal to improve the health outcomes for 10,000 individuals living with diabetes by 2030. In 2024, the Stockton efforts reached approximately 13,000 people, with about 2,000 engaged in diabetes prevention programs.

Abbott emphasizes collaboration with local community organizations and healthcare providers who are trusted within the communities they serve. This partnership model is integral to the company's broader 2030 Sustainability Plan, which aims to advance health equity.
