Artificial intelligence (AI) is increasingly being integrated into the U.S. healthcare system's billing and coding processes, raising concerns about potentially higher costs for patients. A recent analysis by the Blue Cross Blue Shield Association (BCBSA) estimates that AI-assisted medical coding has contributed to nearly $1 billion in additional costs for hospitals between 2023 and 2025.

The BCBSA attributes a significant portion of these increased costs to new diagnoses that move patients into higher reimbursement categories. These diagnoses, often identified by AI based on single laboratory values, may not reflect changes in patient care but still drive up billing. Experts note that AI appears to be accelerating the capture of existing billing incentives within the system.

While AI can assist clinicians with administrative burdens and ensure accurate coding, there is a risk that it could exacerbate the already high expenses within the U.S. healthcare system. Health economist Christopher Whaley suggests that AI makes it easier to capture these underlying billing incentives, even if the additional diagnoses do not clinically impact patient care.

Consumers may face the consequences through higher insurance premiums and out-of-pocket costs. The American Hospital Association has pushed back on the BCBSA's analysis, arguing it lacks context and highlighting AI's role in appropriately capturing patient conditions for care planning.
