The Alzheimer Forschung Initiative (AFI), a non-profit organization dedicated to advancing research into Alzheimer's disease, has published practical guidance for relatives and caregivers on how to manage aggressive behavior exhibited by patients.

The initiative explains that aggressive reactions in Alzheimer's patients often stem from the disease's impact on memory and cognition, leading to anxiety and insecurity. Feelings of dependence and helplessness can also contribute, as can perceived threats from unfamiliar situations, people, or sensory overload such as loud noises or bright lights.

AFI advises caregivers to understand that the patient's behavior is a symptom of the disease, not a personal attack. Taking breaks to regain composure is recommended when situations become overwhelming.

Structuring the patient's daily routine and providing a stable, familiar environment are crucial for reducing anxiety. AFI suggests identifying and mitigating triggers for aggression, such as sensory disturbances, and avoiding direct confrontation or correction, which can escalate distress.

Effective communication involves speaking slowly, clearly, and using simple language and gestures. Asking yes/no questions can be helpful. As cognitive and language abilities decline, maintaining eye contact and physical closeness becomes increasingly important for connection and reassurance.
