Arosa, a care management company, has released a new guide designed to help families recognize and prepare for common challenges encountered after a patient is discharged from the hospital. The company notes that the transition home following a hospital stay or rehabilitation can present unexpected difficulties, even as it represents a relief for the patient.
Care management experts emphasize that discharge instructions may not fully address the practical realities of managing medications, follow-up care, home safety, and daily routines. According to the Centers for Medicare & Medicaid Services, approximately 20 percent of Medicare patients are at risk for readmission after discharge. Families often find themselves responsible for coordinating multiple aspects of care while supporting a loved one's adjustment to new physical limitations or routines.
"Families often believe they are prepared until they get home and realize how many details they need to manage," said Heather Stewart, RN, Arosa Director of Nursing. "Understanding the most common gaps can help families recognize where they may need additional guidance or support." The guide identifies seven common challenges: misunderstanding discharge instructions, confusion surrounding medication changes, difficulty scheduling and managing follow-up appointments, overlooking changes in health or warning signs, mobility issues creating home safety concerns, family members becoming overwhelmed by caregiving responsibilities, and breakdowns in communication among the individual, family, and healthcare providers.
Arosa advises families to proactively ask questions about medications, follow-up care, warning signs, and activity restrictions prior to discharge. They also recommend assessing the home for safety and determining who will assist with meals, transportation, personal care, and medication reminders. The company offers care management, in-home caregiving, and concierge nursing services to support families through these transitions.
