Blue Cross Blue Shield of Arizona's collaboration with Nest Health is expanding access to care for Medicaid members by delivering comprehensive healthcare directly into homes. The program has shown measurable results in closing care gaps and reducing ER visits.
Blue Cross Blue Shield of Arizona (AZ Blue Health Choice) has demonstrated measurable success in enhancing healthcare access for its Medicaid members through a year-long partnership with Nest Health. The program brings comprehensive, whole-family care directly into members' homes, addressing common barriers such as transportation, work schedules, and childcare.
The initiative, which now serves over 2,100 enrolled members, provides medical, behavioral, and social care during a single in-home visit. This model aims to deliver more consistent and coordinated care. Data indicates that enrolled members are closing care gaps at 1.7 times the rate of non-enrolled members, and 88% of enrolled children are up-to-date on well-child visits.
Furthermore, the program has shown a significant reduction in avoidable emergency room visits, with 85% of clinical care line calls that might have led to an ER trip being resolved safely at home. Members also benefit from same- or next-day appointments and 24/7 clinical support at no additional cost.
AZ Blue Health Choice CEO Dr. Heather Carter stated that the model effectively meets members where they are, leading to better outcomes and reduced utilization of emergency care. The insurer continues to focus on reimagining healthcare delivery for Arizona families through accessible and convenient solutions.
