BigHeart Health Launches Free Program for Illinois Medicaid Members

BigHeart Health, formerly MyOwnDoctor, has introduced "My Health. My Power.," a free program established for Illinois Medicaid members. This initiative aims to provide preventive healthcare, personalized support, and connections to social services, intending to help individuals maintain wellness before the onset of serious illnesses.

The program was established after the State of Illinois selected BigHeart Health as the state’s designated Medicaid Prevention Care and Education Organization. In collaboration with the state's Managed Care Organizations (MCOs), the program is projected to engage over 400,000 Medicaid members. Its objectives include closing gaps in preventive care, improving overall health outcomes, and addressing health disparities within the state.

"My Health. My Power. comes at a critical time amidst Medicaid rule changes and potential cuts. Preventive care has never been more important," stated Cheryle Jackson, CEO of BigHeart Health. The program integrates healthcare navigation and education with a focus on addressing social needs. This helps members connect not only with medical providers and preventive services but also with resources for essential needs like food, housing, transportation, and employment.

The program prioritizes Medicaid members who encounter the most substantial barriers to accessing care. This includes individuals with chronic health conditions, families with young children, underserved communities, pregnant and postpartum individuals, those experiencing housing instability, LGBTQ+ members, and individuals with behavioral health needs. Each participant is assigned a Personal Healthcare Consultant to assist with finding healthcare providers and developing a personalized health plan.
