Brilliant Care Model Integrated into New CMS Continuous Care Program

Brilliant Care's nurse-led, AI-enabled care model has been selected as part of the Centers for Medicare & Medicaid Services' (CMS) new longitudinal care program. This collaboration is expected to expand the model's reach to patients managing cardio-kidney-metabolic conditions.

The model has demonstrated clinical results showing an average reduction of 21 mmHg in systolic blood pressure and a 2.56 point decrease in HbA1c. These outcomes provide a proven foundation for the model's effectiveness in managing chronic diseases and support CMS's objective of improving long-term patient care.
