Brook.ai and SRHO Partner for AI-Enabled Continuous Care at 275+ Hospitals

Brook.ai, a company focused on remote care solutions, and SRHO – The National Association of Community Health Organizations – have announced a partnership aimed at expanding the reach of AI-enabled continuous care infrastructure.

The collaboration will support the accelerated adoption of remote patient monitoring within SRHO's member health systems. This initiative is set to extend Brook.ai's Agentic AI platform to hospitals across the United States, impacting over 275 facilities.

Brook.ai's technology provides healthcare providers with continuous, proactive patient oversight outside traditional clinical settings. The aim is to enable earlier interventions and improve patient outcomes by identifying health changes promptly.
