CMS reduces proposed cuts for orthopedic procedures in 2027 Physician Fee Schedule

The Centers for Medicare & Medicaid Services (CMS) has finalized its 2027 Medicare Physician Fee Schedule, implementing a nearly 50% reduction to previously proposed cuts for total knee, hip, and shoulder arthroplasty procedures. The adjustment follows significant pushback from medical societies, lawmakers, and individual practitioners who argued that the original proposals would threaten patient access to musculoskeletal care.

The debate over reimbursement rates occurs against a backdrop of existing workforce shortages, with over 22 million Americans currently residing in counties lacking an orthopedic surgeon. Industry advocates have long contended that aggressive payment reductions create barriers to care, particularly in rural or underserved areas where independent practices operate on thin margins.

AAOS President Dr. Wilford K. Gibson acknowledged the development as meaningful progress but emphasized that systemic issues remain. "There is more work ahead to build a Medicare payment system that reflects the realities of delivering high-quality musculoskeletal care," Gibson said, noting that the organization will continue lobbying for policies that support sustainable practice operations.

The final rule, which takes effect on January 1, 2027, concludes a period of regulatory uncertainty for orthopedic providers. While the mitigation of cuts provides immediate relief, the industry continues to push for broader legislative reforms to stabilize the Medicare payment structure.
