Emplify Health and Pearl Health Partner to Expand Accountable Care in Upper Midwest
Emplify Health and technology firm Pearl Health have formed a partnership to enhance care for approximately 25,000 Medicare beneficiaries across the Upper Midwest, starting in 2027.

Emplify Health, a non-profit health system, has announced a partnership with technology company Pearl Health. The collaboration aims to improve healthcare delivery for approximately 25,000 Medicare beneficiaries across the Upper Midwest, encompassing the networks of Bellin Health and Gundersen Health System.
The partnership, set to commence in early 2027, focuses on supporting older adults to remain healthy and independent within their communities. Pearl Health's accountable care infrastructure includes modeling and managing financial risk, monitoring network performance against Medicare benchmarks, and leveraging insights from providers operating in similar markets.
Emplify Health patients will maintain their current physicians and insurance benefits. Changes will occur behind the scenes: Pearl Health's predictive analytics tools will provide care teams with a more timely view of patient health status. The system will identify patients with rising risk, those who have fallen out of touch with their care team, and where interventions will be most impactful.
Bellin Health Partners ACO ranked second in quality and patient experience out of 482 organizations in 2022. This partnership is expected to sustain performance as Emplify Health assumes greater financial accountability. Both organizations will begin onboarding practices this fall ahead of the January 1, 2027 start date.
