FDA Approves Expanded Use of Pfizer Prostate Cancer Combination Therapy

The U.S. FDA has approved the use of Pfizer's Talzenna and Xtandi combination therapy for the treatment of patients with metastatic prostate cancer at an earlier disease stage.

2 November 2026

The U.S. Food and Drug Administration (FDA) has expanded the approved indication for Pfizer’s combination therapy of Talzenna (talazoparib) and Xtandi (enzalutamide). The treatment is now cleared for patients with metastatic prostate cancer harboring homologous recombination repair (HRR) gene mutations who have not previously undergone androgen pathway modulation.

The approval follows data from the Phase 3 TALAPRO-3 clinical trial, which demonstrated a 52 percent reduction in the risk of disease progression or death compared to Xtandi alone. The clinical benefits were observed across both BRCA and non-BRCA mutation subgroups, providing a new option during early-stage metastatic intervention.

Pfizer executives noted that the decision builds on the existing role of these drugs in advanced oncology. Neeraj Agarwal, lead investigator for the trial, emphasized that the findings underscore the importance of biomarker testing at the time of initial diagnosis for these patients. While 51 percent of patients in the treatment group reported grade 3 or higher anemia, the company stated that adverse events were generally manageable through clinical intervention and supportive care.
