Lightbeam Health and ChartSpan Partner to Unify Remote Patient Monitoring
Lightbeam Health Solutions and ChartSpan have partnered to integrate device-based and deviceless remote patient monitoring (RPM) within value-based care. The collaboration aims to address fragmented data and workflows.

Lightbeam Health Solutions and ChartSpan have announced a strategic collaboration to combine device-based and deviceless remote patient monitoring (RPM) capabilities for value-based care. The partnership aims to provide risk-bearing healthcare organizations with a scalable, multi-modal approach to patient oversight.
The joint offering integrates Lightbeam's analytics and population health platform with ChartSpan's enterprise infrastructure for personal health data. This integration standardizes the ingestion of device-based RPM data across an organization, creating a more unified strategy for patient monitoring.
"Healthcare organizations are integrating personal health data in every direction. That's not a program problem; it's an architecture problem," stated Drew Schiller, Chief Development Officer at ChartSpan. "Our partnership brings together the right capabilities to solve it: Lightbeam's population health intelligence and our enterprise infrastructure for personal health data."
This collaboration enables the embedding of continuous patient monitoring directly into population health workflows. The objective is to reduce avoidable utilization through proactive monitoring and real-time alerting, contributing to more efficient and integrated patient care within value-based models.
