Medicare CJR-X mandate to shift $392 million in hospital costs by 2028

Beginning January 1, 2028, the Centers for Medicare & Medicaid Services (CMS) will implement the CJR-X model, requiring 2,217 hospitals to take on financial risk for joint replacement procedures. CMS estimates a net transfer of $392 million from hospitals to Medicare over five years, as facilities face pressure to manage costs within 90 days post-discharge.

The policy expands on previous mandates where a majority of participants failed to meet target price goals. With more patients opting for elective surgery outside of traditional hospital settings, providers are increasingly responsible for managing post-operative recovery to avoid penalties and qualify for incentive bonuses.

Technology providers are positioning AI-enabled tools as a solution to reduce these financial risks. ROMTech, a Connecticut-based medical technology company, offers a remote rehabilitation system designed to monitor patient progress. CEO Peter Arn stated that the use of data-dense rehabilitation metrics can help predict recovery outcomes early, theoretically reducing readmission rates and overall episode spending.

As hospitals face stricter oversight, the industry is shifting toward remote monitoring to manage patient recovery at home. Providers seeking to mitigate the impact of the CJR-X model are evaluating diagnostic and therapeutic technologies to maintain quality scores and contain costs.
