Operation Pathways Receives Thome Aging Well Grant for Baltimore Program

Operation Pathways has been awarded a 2026 Thome Aging Well Grant to implement its "Strengthening Food Access for Older Adults" program in Baltimore, Maryland. The organization was among 24 entities in Maryland and Michigan that shared $3.8 million to assist lower-income older adults in aging safely and with dignity.

The grant aims to enhance food security for residents at The Bolton North Apartments and The Terraces at Park Heights in Baltimore. Operation Pathways, a subsidiary of The NHP Foundation, provides on-site services designed to help residents age in place, reduce isolation, and maintain stable housing.

Funds from the Thome Aging Well Grant will enable local Resident Services Coordinators to expand SNAP enrollment and renewal assistance. The program will also increase access to food through on-site pantries and food box deliveries, and foster community through resident-led shared meals. This initiative addresses food insecurity faced by over 45,000 seniors in Maryland due to fixed incomes, rising costs, food deserts, and transportation challenges.

Launched in 2022 by Enterprise Community Partners with funding from the Edward N. and Della L. Thome Memorial Foundation and Bank of America, the Thome Aging Well program utilizes a trust-based philanthropic approach to rapidly address community needs. To date, the program has granted over $23 million to 66 organizations, supporting more than 14,000 older adults with services including home modifications, workforce training, health screenings, and food access programs.
