Toledo, Ohio – Wildwood Surgical Center LLC, operating as Wildwood Surgical Center, has announced a data security incident that may have compromised the personal and protected health information of certain individuals. The company detected suspicious activity on its network on June 26, 2025, and immediately initiated an investigation.
Following a thorough review, Wildwood determined that unauthorized access and acquisition of data from its network occurred between June 24, 2025, and June 26, 2025. A detailed analysis of the affected data was completed on May 13, 2026, confirming the presence of personal information (PI) and protected health information (PHI).
The compromised data varied by individual but may have included names, Social Security numbers, government identification numbers (such as driver's license or passport numbers), dates of birth, medical treatment and diagnostic information, health insurance details, and medical billing information, potentially including bank account or payment card numbers. Affected individuals for whom contact information is available will receive formal notification by mail in the coming weeks.
Wildwood states it took immediate steps, including taking parts of its network offline and engaging specialists, to secure its environment and restore operations. Federal law enforcement has been notified. The center advises impacted individuals to remain vigilant against identity theft and fraud by reviewing financial account statements and credit reports, and suggests utilizing free credit reports from major bureaus.
